A token of our appreciation:
We appreciate the time and effort required to answer our questions completely and accurately. If you complete the survey, as a token of our appreciation you will be mailed a $20 gift card to your choice of:
- Tim Hortons,
- Canadian Tire,
- Walmart,
- Subway Restaurants, or
- President’s Choice – Loblaw Ltd. Stores e.g., SuperStore, Zehrs, No Frills, Loblaws)
At the end of the survey, you will be directed to a separate form to complete with your name and address for the purpose of mailing your gift card. This information will be stored completely separate from your survey answers, to maintain the anonymity of your responses. Therefore, nobody will be able to link your name or address with your answers. Only one gift card per person, and gift cards can only be mailed to locations within Canada. You can also opt not to receive the gift card if you prefer not to provide us with your name and mailing address.
If your partner completes the survey, they are also eligible to receive a gift card of their choice.
No gift card?
If you did not get asked to provide your contact information to receive a gift card after completing the survey, we need to hear from you, so we can send you one.

- a couple of questions about when you completed the survey,
- your choice of gift card, and
- your mailing address.
